Letter of Good Standing Request


  1. All CDHEMSS providers requesting a Letter of Good Standing (LOGS) shall do so in writing to the EMS office (email justin.williams@nm.org) .  The requesting provider shall include the targeted EMS system who shall receive the LOGS. Additionally please note if you will remain in CDH's EMSS and at what level (primary or secondary). No letter will be given directly to the EMS provider, rather, it will be sent to the Resource System.
  2. Contact your EMS Coordinator and have him or her send a copy of all your CE hours to the EMS office.  This will need to be completed PRIOR to your letter being sent.
  3. Please allow 2 weeks for you LOGS to arrive at the new EMSS
