Letter of Good Standing Request


  1. All CDH EMSS providers requesting a Letter of Good Standing (LOG) shall do so in writing to the EMS office using the letter of good standing request form (click here for the form).  The requesting provider shall include the targeted EMS system who shall receive the LOG. Additionally please note if you will remain in CDH's EMSS and at what level (primary or secondary). No letter will be given directly to the EMS provider, rather, it will be sent to the Resource System.
  2. Please allow 10 business days for you LOG  to arrive at requested location.
